I take insulin for diabetes — the MCSA-5870 rule, the 45 days, the 12 months

SHORT CERT LIKELY — 1-YEAR

The $49 Medical Card Kit — $49, one-time payment

The checker reads your verdict. The Kit gets you through the exam: documentation packets for your condition, the two 45-day form clocks run as a calendar, the official medication form packet, waiting-period calendars, failure recovery, and the examiner phone script.

What the federal text says

Since 2018, a driver with insulin-treated diabetes qualifies without any exemption when: the clinician who treats the diabetes completes the Insulin-Treated Diabetes Mellitus Assessment Form, MCSA-5870, and the medical exam takes place within 45 days of that signature; the insulin regimen is stable; and the driver has had no severe hypoglycemic episode in the preceding 12 months (an episode requiring another person's assistance, per §391.46). With at least 3 months of electronic glucose records (meter or CGM downloads), the certificate runs up to 12 months; without the records, 3 months. There is no federal diabetes exemption program anymore — FMCSA removed it in 2018 when this rule replaced it. Any source telling you to apply for one is working from a dead program.

What that means for you

Insulin is a paperwork-and-timing test now, not a disqualification. The three things that decide your outcome: the form signed in time, no severe low in 12 months, and 3 months of glucose records — the records alone are the difference between a 12-month card and a 3-month card.

Certificate this points to: 1-YEAR certificates, while the standard's monitoring continues.

What moves the outcome

Book the treating clinician first, the exam second, inside one 45-day window. Download and keep your glucose records continuously — starting the records clock today is the single highest-value move an insulin driver can make. The kit turns both clocks into a dated checklist.

Which mode is this?

This question is decided by the federal text itself. The verdict above is computed from the cited words — your examiner is bound by the same text.

Only a certified medical examiner on the National Registry can certify you. FreightVerdict reads the published federal standards — it does not certify, diagnose, or treat, and nothing here is medical advice.
